How to Register

Simply ask at reception. 

The receptionist will give you forms to complete or the forms can be downloaded and printed below. We ask you to complete these because your medical records may take several months to reach us, and it is essential that we have some details about you.

At the time of registration you will be given the name of your new doctor. To aid continuity of care, we would encourage you to see this doctor at all times unless he or she is unavailable. If you prefer to see a different doctor, either because you do not wish to see your usual doctor or for one particular problem another doctor is appropriate, please explain to the receptionist.

Please let us know of any change of address or telephone number.
