Repeat Prescriptions

How to order your repeat prescription

• the patient or his/her representative (not a contractor/chemist) must request the repeat prescription - no more than 10 days before it is due (unless there are exceptional circumstances)
• requests must be in writing (on white tear-off slip of prescription)
• items required must be clearly 'ticked'
• if white tear-off slip is lost, you will be required to complete a request form from dispensary
• a minimum of 2 WORKING DAYS notice is required
